Post-Mortem: Zero-Token Completions and Upstream Recitation Faults
"Executive Summary During a scheduled workload migration across our multi-model inference gateways, downstream document-processing services experienced a sile"
Executive Summary
During a scheduled workload migration across our multi-model inference gateways, downstream document-processing services experienced a silent failure mode. Upstream foundation model endpoints returned successful HTTP 200 OK responses with valid metadata envelopes, but completely empty payload content (completion_tokens: 0). The root cause was identified as an unhandled edge condition within upstream alignment filtering: a heuristic trigger labeled finish_reason: "recitation".
Because our internal gateway proxy mapped any non-error HTTP status directly to downstream consumers without asserting completion non-emptiness or parsing provider-specific terminal reasons, downstream consumers interpreted the responses as valid zero-length strings. This caused silent drops in processing pipelines, schema validation panics, and unrecoverable data pipeline stalls, while consuming prompt token quotas without generating actionable output.
Incident Telemetry and Payload Anatomy
The incident was isolated to interactions interfacing with provider endpoints utilizing speculative evaluation heuristics. The raw egress response payload captured by our observability proxies illustrates the failure envelope:

}
],

}
Three properties of this payload warrant technical scrutiny:
- HTTP Status Decoupling: The provider daemon returned an HTTP 200 status code despite terminating generation before emitting a single token.
- Resource Consumption: The prompt consumption phase fully executed, consuming 2,298 prompt tokens. Cost and compute were incurred on the inference hardware prior to the safety cutoff.
- Semantic Ambiguity: The
choices[0].finish_reasonfield emitted"recitation"instead of canonical states such as"stop"or"length". Downstream serialization layers had no schema definition for this enum variant.
Chronology of Events
- 14:02 UTC - Traffic redirection begins; 25% of background extraction workers shift to the secondary model tier (
gemini-3.8-flash-n). - 14:07 UTC - Ingestion telemetry alerts on an abnormal drop in downstream message payload byte-sizes ( egress payload size dropped from 1.4 KB to 84 bytes).
- 14:12 UTC - Downstream schema validation alerts fire across worker nodes due to non-nullable document summary fields receiving empty strings (
""). - 14:21 UTC - On-call engineers identify that gateway request counters show 100% HTTP 200 OK statuses, masking upstream application-layer dropouts.
- 14:38 UTC - Log aggregation surfaces anomalous termination strings (
finish_reason: "recitation") across 4.2% of routed requests. - 14:50 UTC - Circuit breaker activated; traffic reverted to primary deterministic model pools. Ingestion backpressure stabilizes.
Root Cause Analysis: Semantic Safety Triggers
Large Language Model (LLM) providers employ safety scaffolds to prevent verbatim reproduction of copyrighted or sensitive pre-training corpora. When an upstream model detects that its generated sequence mirrors an indexed source sequence above a predefined overlap coefficient, the generation pipeline immediately truncates generation.
Formally, let denote the sequence of prompt tokens and denote candidate generation tokens at step . Alignment filters monitor an -gram containment function against a reference corpus :
Under normal conditions, this check occurs dynamically during decode steps (). However, when prompts contain extensive source quotations or structured data that closely match training sets, the speculative decoding engine flags the prefix immediately at .
In the payload under review, the engine evaluated the prompt () against internal overlap indexes. Before emitting token , the evaluator marked the output sequence as an imminent recitation violation. The generator flushed an empty buffer, injected finish_reason: "recitation", and terminated the inference loop cleanly from the perspective of the server transport layer.
The critical breakdown occurred in our proxy layer. Our gateway treated HTTP transport success as semantic pipeline success:
By failing to enforce an explicit contract on finish_reason, our architecture allowed safety-aborted calls to bypass standard retry mechanisms.
Gateway Architectural Breakdown
The legacy gateway handler processed responses using the following flow:
# Legacy handler: Blind transport forwarding
async def process_inference(request: InferenceRequest) -> InferenceResponse:
raw_response = await client.post("/chat/completions", json=request.to_dict())
payload = raw_response.json()
# Flawed assumption: If choices exists, output is valid
content = payload["choices"][0]["message"]["content"]
return InferenceResponse(content=content)
This implementation suffered from two systemic flaws:
- Absence of Terminal Assertion: It lacked assertion checks on
finish_reason. Upstream providers treat non-standard terminations as edge-case completions, but downstream systems require deterministic payloads. - Swallowed Failures: Downstream retries were suppressed because the client received an HTTP 200. The orchestrator believed the job completed successfully, preventing retry or fallback pathways from activating.
Defensive Engineering and Remediation
To prevent unhandled provider states from compromising pipeline integrity, we updated the proxy parsing engine to validate content length and provider termination reasons prior to resolving downstream futures.
from enum import StrEnum
from typing import Any, Dict
import structlog
logger = structlog.get_logger()
class ValidFinishReason(StrEnum):
STOP = "stop"
LENGTH = "length"
TOOL_CALLS = "tool_calls"
class UpstreamInferenceError(Exception):
"""Raised when upstream completes transport without valid semantic content."""
def __init__(self, reason: str, usage: Dict[str, Any]):
super().__init__(f"Inference terminated abnormally: {reason}")
self.reason = reason
self.usage = usage
def parse_and_validate_payload(data: Dict[str, Any]) -> str:
choices = data.get("choices", [])
if not choices:
raise UpstreamInferenceError("empty_choices", data.get("usage", {}))
choice = choices[0]
finish_reason = choice.get("finish_reason")
content = choice.get("message", {}).get("content", "")
# Explicitly isolate safety cutoffs from normal terminations
if finish_reason not in ValidFinishReason._value2member_map_:
logger.error(
"upstream_semantic_fault",
finish_reason=finish_reason,
usage=data.get("usage"),
response_id=data.get("id"),
)
raise UpstreamInferenceError(str(finish_reason), data.get("usage", {}))
if not content and finish_reason == ValidFinishReason.STOP:
logger.warn("empty_content_with_stop", response_id=data.get("id"))
return content
By raising an UpstreamInferenceError, the gateway forces the orchestrator into a structured retry pattern. When a "recitation" or "content_filter" status is recognized, the system routes the prompt to an alternate provider or mutates generation parameters (such as raising temperature or adjusting top-) to break determinism and avoid identical alignment triggers.
Action Items and Forward Posture
- Contract Hardening: Update OpenAPI schemas across all internal proxies to explicitly declare
finish_reasonvariants as typed enumerations. - Cost and SLA Attribution: Update cost accounting routines. Tokens consumed during non-recoverable safety drops must be isolated into dedicated observability dimensions to facilitate upstream provider SLA reconciliations.
- Synthetic Recitation Probes: Introduce chaos integration tests that pass known verbatim prompts to verify gateway fallback behaviors against strict provider alignment mechanisms.
- Telemetry Metric Decoupling: Add dedicated Prometheus gauge metrics tracking non-standard termination states (
model_finish_reason_total{reason="recitation"}) to alert engineers before downstream consumers experience silent data truncation.
